New participants and any athletes who have had COVID-19 need will need to obtain a proper physical and complete the required paperwork. Please visit the links below or come by Sierra Middle School for the required paperwork. 

• Physical Forms
• NFHS Concussion Course (Required by all athletes each year)
• COVID-19 Medical Clearance Form (For any athlete that has tested positive for COVID-19)

If you have a physical from last school year and you HAVE NOT had COVID-19, you can use the physical on file, but the additional paperwork below needs to be completed.

• Physical with Waiver (Physical is on File)
